Mayor's Office Proclamation Request

Please fill out the following information to begin the Proclamation Request process. If the proclamation does not have a direct correlation City of Scottsdale, the proclamation request will be declined. The Office of the Mayor reserves the right to modify proclamation verbiage or deny a request for a proclamation. Requests should be made at least 14 business days in advance of the date the proclamation is needed. Proclamations will not be issued for events or organizations with no direct relationship to the City of Scottsdale, matters of political controversy, ideological or religious beliefs, or individual persons. All proclamation requests must be in writing and submitted via this form.
