2008 State and Federal Income Tax Form Information
| The City of Scottsdale provides both Arizona State and Federal (IRS) income tax forms. Forms are generally available at our offices from February to April each year. For forms and information at other times during the year, contact the tax agencies directly at the locations, phone numbers and websites listed below. |

Most forms are provided free of charge; however, copy fees may apply for multiple copies of forms or instruction booklets. Due to limited space, only the most often-used forms are stocked, and tax forms are limited to those on hand.
Please be aware that City staff are prohibited from providing tax assistance, tax advice, or advice on which forms to file. For tax advice, contact a certified tax preparation service.
Arizona State Tax Forms
Arizona State Tax Forms are available at all four Scottsdale Public Libraries. (View Hours and Locations)The libraries have a limited supply of the following forms:

- 140EZ
- 140A
- 140 PY (Part-Year Resident)
- 140NR (Non-Resident)

Federal (IRS) Tax Forms
Most common Federal forms and instructions are available at two Neighborhood Resource Centers in Scottsdale. Call (480) 312-3111 for hours and locations.

- Visit the Phoenix IRS office and pick up the forms in person. The office is located at 210 E. Earll Drive. Office hours are Monday through Friday from 8:30 a.m. to 4:30 p.m. Call (602) 207-8031 for recorded information or to schedule an appointment for assistance.
