What Do I Need to Obtain a Passport?

New Applicants:  

  • Two official passport photos (2"x2")
  • Certified Birth Certificate or Expired Passport
  • Driver's License or other ID
    (please call to confirm what is acceptable)

Passport Fees:

  • Passport Book

    Adult (16 years +)

        $110.00 payable to: US Department of State

        $25.00 payable to: City of Scottsdale (execution fee)

    Child (15 years and under)

        $80.00 payable to: US Department of State

        $25.00 payable to: City of Scottsdale (execution fee)

  • Passport Card**

    Adult (16 years +)

        $30.00 payable to: US Department of State

        $25.00 payable to: City of Scottsdale (execution fee)

    Child (15 years and under)

        $15.00 payable to: US Department of State

        $25.00 payable to: City of Scottsdale (execution fee) 

  • Passport payment shall be made by CHECK OR MONEY ORDER ONLY, payable to U.S. Department of State. $25 execution fee may be made by check, cash, money order or credit card, payable to City of Scottsdale.

    **Passport Cards - The passport card will facilitate entry and expedite document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.  The card may not be used to travel by air.  It will otherwise carry the rights and privileges of the U.S. passport book and will be adjudicated to the exact same standards.  For complete information on passport cards visit the U.S. Department of State website.

Photographs:

  • You need 2 identical passport photos size 2x2 taken within the last 6 months. It must be front view with a white or off-white background. (Photographs can be taken at CVS, Kinkos, Walgreens, etc.)

Evidence of Citizenship:

ONE of the following documents MUST be presented:

  • Certified copy of your birth certificate
  • Certificate of Naturalization or of Citizenship
  • Expired Passport
  • Consular Report of Birth
  • For child passport renewals, applicants under age 16 will be required to submit a certified birth certificate AND their previous passport.
  • If you are unable to present acceptable documentary 'IDENTIFICATION" you may bring a person who has known you for more than two (2) years. To be acceptable, the witness must be either an American citizen or permanent resident alien and must present appropriate identification.


Renewal Applicants:


You must renew by mail if you answer yes to ALL of the following statements:

  • You were 16 years or older, when your most recent passport was issued
  • AND you can submit your most recent passport
  • AND your most recent passport was issued less than 15 years ago
  • AND you use the same name as on your most recent passport, or you can submit a certified copy of a marriage certificate or certified copies of court documents showing a legal name change.

If you answered no to ANY of the four questions, you cannot renew by mail. You must apply in person and use the DS-11 application.

 

What you must include when you submit your applicaiton by mail:

  • Your most recent passport
  • Two recent passport photos
  •  Check or Money Order payable to The US Department of State (Full name and birth date must be on front of check/money order)
  • Application for Renewal by Mail, DS82
  • Marriage certificate or court documents if name has changed (original or certified copy required)

Passport Book       $110.00

Passport Card       $ 30.00

Book & Card         $140.00

Expedited service by mail is available. ( Please check www.travel.state.gov under Passport Renewal to find anticipated processing time for expedited service.)

Expedite Fees and Mailing:

Passport Book      $182.72 (includes passport fee, expedite fee, return Express Mail fee)

Passport Card      $ 90.00 (includes passport fee, expedite fee. Return Express Mail not available)

Book and Card     $212.72 (includes passport fee, expedite fee, return Express Mail fee for book)
