The following list of forms may be printed and filed with the Court. All forms can be opened using Adobe Acrobat. NOTE: When filing a criminal motion you must submit a copy of the motion to the City Prosecutor’s office.

Protective Order Petition 

Protective Order Guidesheet and Petition (PDF). Contains instructions and form to petition for an Order of Protection / Injunction Prohibiting Harassment / Injunction Prohibiting Workplace Harassment.  For information regarding the types of protective orders and filing instructions please visit the Protective Order page.

Defendant Guidesheet Form (PDF). Contains form to assist in the service of Protective Orders.

Declaration of Service (PDF). Contains form to declare service of Protective Orders. 
Please note: this form is to be used only by persons authorized under A.R.C.P. 4 or ARS 13-3602(Q) to serve legal papers.

Motions

General Motion Form (PDF). Contains instructions and form for filing a motion with the court.  You can also file a motion online using our Online Court SystemPlease note: when filing a criminal motion you must submit a copy of the motion to the City Prosecutor’s office. 

Documentary Hearing 

Request for Documentary Hearing (PDF). Contains instructions and form for filing a request to have a Civil Traffic Documentary Hearing.

Note:  If you have a parking case, your case needs to be filed with the Court and you need a Scottsdale City Court case number before you can request a documentary hearing.

Criminal Notice of Appeal

Application to Vacate Conviction - 13-907.01

Application to Vacate Conviction - 13-907.01 (PDF)  Contains instructions and form for filing Application to Vacate the Conviction for the crime of Prostitution, under A.R.S. § 13‐3214, committed prior to July 24, 2014.

Request for Accommodation by Person with Disabilities (and Response) 

Request for Accommodation by Person with Disabilities (PDF). Form contains section for person requesting accommodation and section for the Court's ADA Coordinator to respond. 

Subpoena/Subpoena Duces Tecum

Subpoena/Subpoena Duces Tecum (PDF). Form contains section requesting that a person appear in court in testify in a case or is commanded to bring certain documents to a court or administrative agency. 

Customer Complaint Form

Customer Complaint Form (PDF). This form is strictly for the purpose of filing a written complaint on an issue that was not satisfactorily resolved by court staff and is not for the purpose of contesting a judicial decision on a court case.

Court customer complaints are treated seriously and will be addressed by a verbal or written response within five (5) business days. The complaint form is provided for your convenience and may be recorded in the court’s case management system. The City Court has no authority to address complaints pertaining to other City departments and therefore non-court complaints shall be referred to the appropriate department in which the complaint originated.

Walk-in hours (criminal courtrooms): Monday - Friday, 8:30 - 11:30 a.m. and 1:30 - 4:30 p.m.

Hours: Monday - Friday, 8 a.m. - 5 p.m. Closed on city holidays

Except the first and third Wednesday of each month, open 8:30 a.m. - 5 p.m.
