Bingo licenses/permits are issued by the State with city reviewing the proposal and providing a "local governing body endorsement".

Step One

• Complete the state's Bingo Application (pdf / 481 kb / 5 pgs)
• Call the Arizona Department of Revenue at 602-716-7801

Step Two

• Submit COS Bingo Application to Teri Gleason, in Development Services.

Step Three

• Bingo application will be reviewed by the City Council and a recommendation of approval or denial will be forwarded to Arizona Department of Revenue, Bingo Section.
