Aetna High Level CPOSII PPO Overview

The Aetna High Level CPOS II PPO plan offers two tiers of coverage within a single plan design. As a participant, you have the choice of using a contracted network of providers for lower out-of-pocket expenses or you may utilize out-of-network providers and incur higher out-of-pocket expenses, and possibly all billed charges. Participants can choose either tier of benefits at any time. |
| This plan has a $500 in-network deductible and a separate $2,000 out-of-network deductible. |
Provider Network The Aetna CPOS II medical plan has an enhanced network of physicians and other health care providers. Aetna CPOS II members may also receive services from Mayo Clinic Scottsdale and Mayo Clinic Hospital, however the doctors and hospitals will not be listed in the Aetna directories. Aetna Prescription Program Information Retail pharmacy: When you go to the pharmacy, give the pharmacy your ID card. The pharmacy will need the information on your card to check eligibility and to update your records.
